How to register

Anyone wishing to register with our Practice must have an address within the SE7 postcode. If you would like to register with our Practice, we will ask you to provide 2 forms of ID—1 proof of identity and 1 proof of residential address (a list of all accepted documents is available from the reception desk). We accept Registration requests between 12 midday and 4.0pm, Monday to Friday, because this is the quietest time of the day.  Please come to the surgery with your 2 forms of ID, and you will be given some registration forms to complete. We will then make you a registration appointment with Janice, our Healthcare Assistant, or with one of our Practice Nurses.

Named Accountable GP
Every patient who registers with The Fairfield Practice is allocated a Registered Doctor.
From 1st April 2015, this Doctor will be known as your ‘Named Accountable GP’. This GP will be responsible for your overall care at the Practice.
This does not prevent you from seeing any GP in the Practice, or from speaking to the Duty Doctor for any urgent problems/advice as you currently do.
If you cannot remember or would like to check who your Named Accountable GP is, please ask the Receptionist.
